Things to bring to your visit

  • A list of all medications you are taking and their dosages.

  • Copies of any recent laboratory test results, or the results of any significant medical diagnostic tests, even if they were negative ( i.e., if the results were normal) which may be relevant to your current complaint(s).

  • Your current insurance information (ID number and mailing address for claims). Although we DO NOT accept your insurance as payment directly, (view our FAQ's) occasionally circumstances arise where this information is necessary. Unless necessary, the information will simply be retained on file.

  • Bring a form of payment (check, cash, Visa or MasterCard). Payment is collected on the date that services are provided. Since the situation may arise on any day that you require some special service in addition to your consultation with the doctor, IT IS ALWAYS SAFEST TO BRING ALONG AN ADDITIONAL FORM OF PAYMENT that may be used as needed to directly pay a laboratory, or other service provider (i.e. an extra check, extra cash, etc.).

  • You may wish to bring a notepad to take notes, or an audio cassette tape to record your session. This type of medicine is really more like an education, and just like a lecture in school, you may need to hear the same information repeated several times before it really “sinks in.”

  • If another family member or friend may be helpful in providing parts of your history, cares for your health, or may provide a second useful listening ear during your session, please feel free to include them in our first and/or subsequent visits.
